Heartland Hospice is a compassionate approach to caring for those who are faced with a life-limiting illness. Hospice provides patients and their families with supportive services that not only keep the patient comfortable and educate the family, but also provide emotional support.
Many people think that hospice is a place, but really it is a philosophy of care. Whether a patient is living in the home he or she has owned for years, a nursing home or assisted living, hospice care can be tailored to fit the patient’s and family’s unique situation.
The typical hospice team consists of the patient’s physician and the hospice physician; registered nurses; social workers; spiritual care coordinators; bereavement counselors; dietitians; pharmacists; physical, occupational and speech therapists; home care aides; and volunteers. Some patients even benefit from music, art and massage therapists, as well.
Access to hospice is based upon a documented life-limiting prognosis by a certified physician and the patient’s decision to elect to receive hospice benefits. Hospice is a 100 percent-covered benefit under Medicare. Hospice care is also covered under Medicaid and many insurance plans.

Northern Illinois Hospice and Grief Center is a non-profit, tax-exempt organization and is licensed by the Illinois Department of Public Health as a comprehensive hospice and is Medicaid and Medicare certified.
Our tradition of caring ensures that we:
- Accept patients from many referral sources.
- Provide respectful and superior healthcare.
- Help patients live fully and comfortably.
- Provide an expert team of professionals for each patient.
- Support patient and family decisions.
- Offer bereavement services for family and friends for a minimum of 13 months following the death of the loved one.

Hospice is for patients with a life-limiting illness, with a prognosis of 6 months or less to live, who have decided not to pursue curative or life prolonging treatment and instead want to focus on palliative (comfort) treatment. Anyone diagnosed with a terminal illness can be eligible for Hospice. It is not just for cancer, but also for end-stage heart and lung disease, kidney failure, AIDs, Alzheimer's, etc.
Hospice is a concept of care for patients of all ages, allowing them to live their last days in an atmosphere of loving kindness surrounded by loved ones. It is available to patients at home or in a nursing facility.

The Haven Network strives to meet the needs of these grieving families and if possible, become involved in their journey at the time they receive the diagnosis of their pre-born baby’s terminal condition. We are an independent community organization that collaborates with physicians and clergy to bring a caring and sensitive response to broken-hearted families.
Our Family Services Program provides emotional, spiritual and practical support, including meal delivery, layettes and burial gowns, and memory boxes through trained volunteers. Our Family Partner volunteers receive specialized education that assists them to journey with their assigned family for one year and will extend that support through subsequent pregnancies if desired.
The Haven does not charge for its services but relies on charitable donations and grants.

Visiting Nurses Association's Hospice offers specialized treatment and compassionate care for patients and families coping with a terminal illness. VNA Hospice's focus is to assist caregivers in offering supportive services to allow patients to live in their choice of residence with the greatest degree of comfort, dignity and control. Hospice commits to helping patients and families cope with illness and maintain or enhance the quality of life. Hospice affirms life. It neither hastens nor postpones death. Hospice is for patients who have chosen to no longer seek curative treatment. Rather, they choose a plan to be free of pain and symptoms and to attain a mental and spiritual preparedness for death.
Services are provided through a holistic team approach of professionals and volunteers. VNA is licensed to provide hospice services in the following counties: Winnebago, Boone, western McHenry, and Ogle.
