April Events

  • Making Choices Seminar, Tuesday, April 13, 2010 : 10:00 AM to 12:00 PM
Grace Funeral & Cremation Services, is hosting a “Making Choices” seminar, in conjunction with the staff of Heartland Hospice. A Registered Nurse and Licensed Social Worker on staff with Heartland Hospice will provide information about Durable Power of Attorney for Health Care and Living Will Documents, as well as present information on the “Making Choices” planning guide. Stateline residents are encouraged to attend the event at Grace Funeral & Cremation Services, 1340 South Alpine Road; Rockford – at Colonial Village on April 13, 2010 at 10 am. The “Making Choices” seminar is a tool for the public to use when talking about their health care wishes with their family, friends and health care providers. Heartland Hospice will offer a complimentary “Making Choices” planning guide that helps patients and family members open discussions about their health care decisions. “Our hope is that this brochure will help more people in the community to plan ahead for their health care,” Tammy Christiansen, RN said. “It’s difficult to have these discussions with family and friends; by planning ahead we are hoping that fewer families and health care providers will have to make difficult decisions in the absence of guidance from the patient. The ultimate goal is to honor the patient’s wishes and the first step is to understand what the patient’s wishes are.” Participants will have time to ask questions about end of life issues, hospice care and will have the option of meeting with a Licensed Social Worker who can assist participants in completing the Durable Power of Attorney for Health Care and Living Will documents. “This event is the first in a series that we have planned for community education. We believe that this topic is an extremely valuable resource for the community to explore,” said Grace Funeral & Cremation Services co-owner, Kristan McNames. “The ultimate goal of Heartland Hospice and Grace Funeral & Cremation Services is to educate the community about services provided by both of our organizations.” There is no charge for this event; light refreshments will be served. Please RSVP by April 9, 2010 to 815-395-0559, or rsvp@gracefh.com.
